If you have purchased Functional Lab Analysis Level II (Labs Included), this package include the lab markers listed below. Within a few days after purchasing this package, you will receive a lab requisition form with instructions and next steps.

If you have purchased Functional Lab Analysis Level I (Bring Your Own Labs) and will supply your own lab tests, you may take the list below to your healthcare practitioner and request these labs. If it would be helpful for you to have a letter of explanation, please request it at the time you’ve ordered this service.)

Please do not eat anything by mouth after midnight. Water is fine however no coffee or tea. Morning thyroid medications are fine to take, however please discontinue supplements at least three (3) full days prior to testing. Testing is recommended to be scheduled first thing in the morning. Bring a healthy snack with you so you can have that immediately after your blood draw.

Rhode Island, New York and New Jersey Residents

Please note that blood draws cannot be performed in the states of New York, New Jersey or Rhode Island at this time as these states do not allow patients to have direct access to labs. You will have to travel to a nearby state to provide your blood samples and have an address in that state for sending results. Providing blood samples in all other states will be fine with no restrictions.
